Health Care Waste Management (HCWM) Trainings and Orientations at two locations: (1) Khadak Municipality (Saptari), (2) Birendranager Municipality (Surkhet), SNV



Background
SNV is implementing the DGIS funded WASH SDG sub-programme in four cities of Nepal:
Khadak, Nepalgunj, Chandannath and Birendranagar. The overall goal of the WASH SDG sub-
programme is to improve health and quality of life of people across all social and ethnic
groups through access to sustainable, inclusive, environmentally safe sanitation and improved
hygiene practices.

As a part of the sub-programme, all four cities have developed City Sanitation Plans (CSP) and
have been using these plans to guide activities and progress on the various thematic areas of
the CSP. One key thematic area is strengthening the practices of health care waste
management. The sub-programme has been engaging in dialogue with three of the local
governments on their capacities and gaps. A summary of the needs identified are as follows:

Khadak Municipality:
A visit to the private health clinics and PHCC showed that the health service providers do not
have a proper waste segregation process at source and the majority of waste is being dumped
and burnt in a pit on premises. In the first week of February, a group of representatives from
the Municipality and the health care service providers visited Sindhuli Hospital which has
established a model system of segregation at source, treatment, and disposal/recycling. Upon
their return, the participants are eager to assess their own situation and develop a plan to help
them take action for improvement. To this end, the need now is to conduct:

- One-day post-exposure trip workshop in Khadak with Municipality technical staff and the health care providers
to capture their enthusiasm of what they learnt from Sindhuli hospital and assess the situation in their own
health facilities and to prepare a practical plan for the way forward.

Chandannath Municipality:
In Chandannath, the hospital has been storing health care waste on its compound; some of this
has been transferred and disposed to the dumping site. While the hospital has tried to segregate
the waste, this waste can still be considered as contaminated because of improper segregation
at point-of-care of infectious waste. The local government officials, hospital staff, and private
clinic operators want to have technical training and exposure to understand the concepts and
practices of health care waste management from source to end disposal/reuse. Therefore, the
need now is to conduct:

- Three-day training (including practical aspects) for Municipality officials and government and private sector
staff, in Birendranagar where they can have exposure to good practices (e.g. in Shining Hospital INF Surkhet),
and accordingly analyse the situation in their own area and develop an action plan to guide them in the way
forward.

Birendranagar Municipality:
In Birendranagar, the Nepal Chemists and Druggist Association (NCDA), in partnership with the
Municipality, established a decontamination center for infectious waste a few years ago. The
Association has organised a system of collection of contaminated waste from private clinics,
autoclaving at the center, and transportation of the treated waste to the landfill site. The
Association has more than 100 private sector medical providers as members. Currently, not all
of them are part of this service. For those that are availing the service, the key challenge is
that they are not segregating waste at source properly so the volumes of contaminated waste
are unnecessarily high. The NCDA has identified the following need:

- Half-day orientation on overall concepts and practice of segregation and management of contaminate waste
for as many of their members as possible.

Scope of Work
SNV Nepal is looking for a Consultant(s) who is experienced (technical knowledge, practical
experience) in health care waste management from source to disposal to support with
implementation of the above identified needs. The Scope of Work includes two packages:

Package 1: Khadak
Activities and time duration
As part of the assignment, the Consultant shall conduct the following activities:

• Conduct one-day workshop in Khadak, with Municipality technical staff and health care providers on building
on their previous practical orientation on HCWM in Sindhuli Hospital to assess the situation in their own health
facilities and to prepare a practical plan for the way forward. The service providers shall prepare a basic plan
for their respective health facilities. The Municipality staff shall prepare a basic plan on how to support and
monitor health facilities in improving the HCWM system in their facilities. 

Package 2: Birendranager
Activities and time duration
As part of the assignment, the Consultant shall conduct the following activities:

• Up to four sessions of half-day orientations for members of NCDA in Birendranagar (each group will be
between 20 – 30 participants) on overall concepts and practice of segregation and management of
contaminated waste, aimed at ensuring that only contaminated waste is sent to the decontamination centre.

• Three-day training, in Birendranagar, on theory and practice of HCWM for team of Municipality officials and
private and public sector health care providers from Chandannath Municipality (approx. 20 people), and an
action plan to guide them in the way forward (basic action plan for each participating health facility; and basic
action plan by Municipality officials on how to support and monitor the health facilities.
