Health Care Waste Management (HCWM) Implementation Project at Crimson Hospital

Health care waste management implementation project at Crimson hospital is a project of Crimson Hospital and Waste Services Pvt. Ltd. as its consultant partner.

Project Overview:

Very few hospitals in Nepal meet basic standards for the segregation, treatment, and disposal of hazardous waste. Recently Nepal government has developed its HCWM Standards and Operating Procedures 2020 as a guiding document for the safe implementation of HCWM system.

For the implementation of HCWM system, there should be detail knowledge over the waste generation pattern, types, volume and their flow as well as current practice of waste segregation, transportation, storage, treatment as well as final disposal. Similarly, level of trainings on HCWM, occupational health and safety features are crucial to understand better about the entire HCWM practice.

It is a challenge to manage waste for a single hospital on its own as it lacks trained human resource, appropriate technology as well as concrete implementation plan. Thus, the main aim of this study is to find out the gaps in overall HCWM system in Crimson Hospital, so that a best suited HCWM implementation plan along with monitoring plan and capacity development plan and list of logistics as well as appropriate technology could be recommended to crimson hospital for the safe, effective, environmental-friendly and sustainable system of health care waste management.

The objectives of the project were:

General Objective:

  • To implement the health care waste management system in Crimson hospital to reduce the public health risk of health care waste.

Specific Objectives:

  • To strengthen good governance in health care waste management through developing standard operating procedure, capacity development by training and coordination with local government and different stakeholders.
  • To develop healthcare waste management plan and employ best practice in hospital.
  • To initiate pilot program in specific ward and to ensure its sustainability. 
  • To develop the monitoring, recording and reporting system waste management and its documentation. 

Location and Duration:

Crimsom Hospital is located at  Tilottoma Municipality Nepal and the project started from third week of October until the November 2021 (45days). 


Technical Support and Leadership: Waste Service Pvt. Ltd., provided technical support, leveraging its expertise to drive the project towards success and Tika Ram Poudel, a recognized leader in waste management, headed the project as the Team Leader.

Activities and outputs of Implementation Plan:

  • Conducted Inception meeting with concerned officials, province, municipality and stakeholders.
  • Provided training to HCWM committee, waste handlers, support staffs, autoclave operators and other health care professionals.
  • Formed HCWM Committee
  • Conducted detailed HCWM assessment
  • Developed HCWM plan in correspondence to HCWM standard operating procedures (SOP).
  • Planned the Transportation, Treatment and Final Disposal of waste to ensure sustainability of the system.
  • Monitoring and evaluation of the implementation plan.
  • Autoclave validation was done to ensure the quality of waste sterilization. The Waste Service employed a biological test indication and a bowie & disk test for the validation process. The autoclave validation was conducted correctly and passed both temperature tests.
  • Recording and reporting system was initiated.
